For new patients. Please download and print the forms. Please fill them in and bring them to your first visit to our office. You can also fax them to our fax line (678) 565 9856 ahead of your visit. This way you can save time with the necessary paper work at your visit.

Pediatric physicals are important screenings pediatricians are performing to ensure that a child is healthy enough to participate in a specific activity like e.g. a sport program. They will be commonly needed starting at daycare age through the teeanage years all the way to adulthood. You can download, print and fill out the form prior to your visit.

We are using the Vanderbilt Teacher Behavior Evaluation Scale to screen patients for ADHD related symptoms. The questionaire is for children between 4-18 years of age. Ideally the forms are answered by more than one source to achieve more objectivity. Like at least one parent/caregiver and the child's teacher(s) should fill in the forms. You can download and print the questionaire as many times you need it. Then fill them in respectively have them filled in. When the forms are complete, you can fax them to us (678) 565 9856  or bring them along when you are coming for your visit. 

If somebody else other than your child's biological parent/legal guardian (e.g. grandparents, other family members) will bring your child to our clinic for medical care, we will need you to complete a consent form.

Please notice that such an authorized "proxy" caregiver must be at least 18 years of age.

If you are switching to us from another medical practice, please use this form to request your child's medical records to be forwarded to our office.

Ticks can be found nearly everywhere in Georgia and almost all around the year. But especially in summer the risk for kids to be biten by a tick is highest. As ticks are carriers of some serious deseases, you should know what to do if your child has been bitten and also how to remove the critter properly in case it is still stuck in the skin.

Poisonous Ivy, Poison Oak, Poison Sumac and other poisonous plants are common in the South East. When kids play outisde they can get in in contact with those plants that can cause severe skin reactions, others should not be ingested. Know how to identify those plants and teach your children to avoid them. If you are suspecting that your child got in touch with one of them we are ready to help - or in serious cases call the Georgia Poison Center 1-800-222-1222.
